Liability Waiver *
Release of Liability & Assumption of Risk I acknowledge that participation in athletic activities involves inherent risk of injury. I voluntarily consent on behalf of my child and agree to hold North Haven Health & Racquet Club, its staff, coaches, and volunteers harmless from any claims arising from participation in the Summer Program 2024, except in cases of gross negligence.
